We specialize in providing care for recipients of Medicare and other Medicare Advantage Plans such as Secure Horizons. Our physicians and staff want to partner with you to provide the highest quality of health care.

Please take a few minutes of your valuable time to fill out our new patient forms. It is very helpful to your doctor on this first visit to have this information available. It will enable your physician to put together a care plan that addresses all of your medical concerns and issues. If you have a question about any part of the form or are uncertain about any of the information, we can help you when you check in.

On your first visit we ask that you arrive 30 minutes before your scheduled appointment time with your new doctor. That way we will have enough time to get all of your paperwork and forms finished, and you won't feel rushed. Please bring your insurance cards as well, so we make sure we have the correct information for billing purposes.

And one final request, please bring either a list of your current medication(s) or all of your pill bottles. Your new doctor wants to review your current medication with you. If it's helpful, we have a handy medication record you can download to list and keep track of your medications.
